Position summary:

The Patient Treatment Coordinator (PTC) serves as the coordinating contact between the Patient and our team of Registered Dietitians and Health Care Professionals.


Responsibilities include:
 

  • Plan, coordinate and manage all on-boarded patients’ treatment schedules.  

  • Ensure the Patient has informed understanding of their Service Agreement.

  • Review the patient’s file to determine appropriateness for treatment based on desired outcomes.

  • Review each Patient file for accuracy, ensure all files are complete, and identify any potential issues that may delay the treatment process.

  • Coordinate the date and time of patient treatments.

  • Liaise with inter-professional team members as required to plan an effective delivery of care.

  • Collect and maintain accurate records during the process.

  • Establish and maintain contact with the patient.

  • Liaise with the patient to provide ongoing support/education during the treatment phase, as required.

  • Communicate patient information to the clinical team and support services as appropriate, to facilitate a safe and efficient treatment schedule.
     

The successful candidate will have the following qualifications/experience:
 

  • Demonstrated flexibility and ability to work under pressure in a fast paced environment.

  • Excellent interpersonal skills and the ability to work co-operatively and constructively within an inter-professional healthcare team.

  • Effective verbal and written communication to successfully work with all levels of the organization.

  • Demonstrated critical thinking/problem solving skills.
