Position summary:
The Patient Treatment Coordinator (PTC) serves as the coordinating contact between the Patient and our team of Registered Dietitians and Health Care Professionals.
Responsibilities include:

Plan, coordinate and manage all on-boarded patients’ treatment schedules.

Ensure the Patient has informed understanding of their Service Agreement.

Review the patient’s file to determine appropriateness for treatment based on desired outcomes.

Review each Patient file for accuracy, ensure all files are complete, and identify any potential issues that may delay the treatment process.

Coordinate the date and time of patient treatments.

Liaise with inter-professional team members as required to plan an effective delivery of care.

Collect and maintain accurate records during the process.

Establish and maintain contact with the patient.

Liaise with the patient to provide ongoing support/education during the treatment phase, as required.

Communicate patient information to the clinical team and support services as appropriate, to facilitate a safe and efficient treatment schedule.
The successful candidate will have the following qualifications/experience:

Demonstrated flexibility and ability to work under pressure in a fast paced environment.

Excellent interpersonal skills and the ability to work co-operatively and constructively within an inter-professional healthcare team.

Effective verbal and written communication to successfully work with all levels of the organization.

Demonstrated critical thinking/problem solving skills.
